During the COVID wave of 2021, there was a time when Dolo 650 was completely out of stock in every pharmacy across India — including mine. Patients were desperate. They would come in asking for Dolo specifically and refuse to believe that Crocin would work just as well.
That experience made me realize how confused people are about these two medicines. So many patients think Dolo 650 and Crocin are completely different drugs — one stronger, one weaker, one better, one inferior.
Today I want to clear this confusion once and for all. As a pharmacist who has dispensed both these medicines thousands of times, I will give you the complete picture — what is the same, what is different, and which one you should actually pick for fever, headache, body pain, and cold.
The most important thing first — both are Paracetamol
Let me say this clearly before anything else. Dolo 650 and Crocin both contain the same active ingredient — Paracetamol. That is it. There is no secret additional ingredient in one that the other does not have. There is no superior formula. They are both Paracetamol tablets.
The difference — and the only real difference between the two most popular versions — is the dose.
Dolo 650 contains 650mg of Paracetamol.
Crocin (standard tablet) contains 500mg of Paracetamol.
Crocin Advance contains 500mg of Paracetamol with a faster release formulation.
Crocin Pain Relief contains 650mg of Paracetamol plus 65mg of Caffeine — this is different and I will explain it separately.
So when someone asks me — "Dolo 650 stronger hai ya Crocin?" — my answer is: Dolo 650 has 150mg more Paracetamol than a standard Crocin tablet. That is the primary difference. Nothing more dramatic than that.
Why did Dolo 650 become so popular?
This is a question I find genuinely interesting from a pharmacy perspective. Paracetamol 650mg tablets have existed for years. But Dolo 650 — made by Micro Labs — became the household name almost overnight during COVID, when it was recommended as the first-line fever management medicine.
Suddenly every doctor was writing Dolo 650 on prescriptions. Every patient was asking for it by name. And a brand that was already popular in South India became a national obsession. Micro Labs reportedly sold billions of rupees worth of Dolo 650 during the pandemic period.
But here is what most people do not realize — dozens of other companies make the exact same 650mg Paracetamol tablet. Calpol 650, Sumo, Fepanil, P-650 — all are 650mg Paracetamol. The medicine is identical. The brand name is different. The price is sometimes different. But the drug? Exactly the same.
Understanding the different Crocin variants
One reason people get confused between Dolo and Crocin is that Crocin comes in multiple versions. Let me break them down simply:
Crocin 500 — Standard tablet. 500mg Paracetamol. For mild fever and headache. This is the one most people think of when they say "Crocin."
Crocin Advance 500 — Same 500mg Paracetamol but with a faster dissolving formulation. GSK claims it works in 15 minutes. The medicine is the same dose but the tablet is designed to dissolve and absorb faster. Good for people who want quicker relief.
Crocin Pain Relief — This one is different. It contains 650mg Paracetamol PLUS 65mg Caffeine. Caffeine enhances the pain-relieving effect of Paracetamol — this combination is well-established in pharmacology. It is better for tension headaches and body aches where the extra kick of caffeine helps. However — avoid this if you are sensitive to caffeine, have heart palpitations, or take it close to bedtime.
Crocin 650 — Yes, Crocin also makes a 650mg Paracetamol tablet. So at 650mg strength, Dolo and Crocin 650 are literally the same dose. Same drug. Different brand. Different price point.
Which one should you take for fever?
For fever — any standard Paracetamol tablet will work. The choice between 500mg and 650mg depends on your weight, age, and the severity of fever.
In my pharmacy practice, I follow this simple guide:
For adults above 60kg body weight — 650mg Paracetamol (Dolo 650 or Crocin 650) is appropriate for significant fever above 101°F.
For adults between 40 to 60kg — 500mg Paracetamol (Crocin 500) is often sufficient for mild to moderate fever.
For elderly patients — I prefer starting with 500mg because elderly kidneys and livers process Paracetamol more slowly. Starting with a lower dose and increasing if needed is safer.
For children — Neither Dolo 650 nor Crocin adult tablets. Children need weight-based dosing with paediatric syrups or drops. Never give an adult Paracetamol tablet to a child without a doctor's guidance.
💊 Pharmacist Tip from Santosh
The most common mistake I see — people take one Dolo 650 and when the fever does not break in 30 minutes, they take another one. Paracetamol takes 45 minutes to one hour to show full effect on fever. And the safe gap between two doses is a minimum of 4 to 6 hours. Taking it too frequently does not lower fever faster — it only increases the risk of liver damage.
Which one is better for headache?
For a simple tension headache — 500mg Paracetamol (Crocin 500) is usually enough. Many people reach straight for Dolo 650 when a standard Crocin tablet would have worked just as well with less Paracetamol load on their liver.
For a migraine or severe headache — Crocin Pain Relief (650mg + Caffeine) can actually work better than plain Dolo 650 because caffeine specifically helps constrict the dilated blood vessels that cause migraine pain. This is the one time I might recommend a Crocin variant over Dolo.
However — if you get frequent headaches and are taking Paracetamol more than 10 to 15 days per month, please see a neurologist. Overuse of painkillers — any painkiller including Paracetamol — can actually cause medication overuse headache, which is a real and frustrating condition I see in patients who have been self-medicating for years.
Which one is better for body pain and cold?
For body pain and the aching that comes with viral fever or flu — 650mg Paracetamol is more appropriate for adults. Here Dolo 650 or Crocin 650 would be my recommendation over the 500mg variants.
For cold symptoms — please understand that Paracetamol only treats fever and pain. It does not treat a blocked nose, runny nose, cough, or sore throat. Those symptoms need different medicines. Many people take Dolo 650 for a cold and wonder why the cold is not getting better. The cold is not getting better because Paracetamol was never meant to treat it — only the fever and body ache that come with it.
The liver warning nobody talks about
This is the part of my job that I feel most responsible about telling people. Paracetamol is extremely safe — when used correctly. But it is one of the most common causes of drug-induced liver damage in the world when misused.
The maximum safe daily dose of Paracetamol for a healthy adult is 4000mg per day — that is four grams. In practice, most doctors recommend not exceeding 3000mg per day to keep a safe margin.
Now do the math. Dolo 650 — three tablets a day — that is 1950mg. Four tablets — 2600mg. Still within limits if taken correctly with proper spacing.
But here is where people go wrong. They take Dolo 650 for fever. They also take a cold medicine that contains Paracetamol. And maybe a combination tablet for body ache that also has Paracetamol. Suddenly they are taking 5000mg or more of Paracetamol without realizing it — because they do not know that multiple products contain the same ingredient.
In my pharmacy I always check — before dispensing any combination cold medicine — whether the patient is already on Paracetamol. This kind of hidden double dosing is a real danger and patients are not aware of it.
🚨 Paracetamol + Alcohol = Serious Liver Risk
If you drink alcohol regularly, even the standard dose of Paracetamol can be dangerous for your liver. Alcohol and Paracetamol together are processed through the same liver pathway and the combination dramatically increases the risk of liver damage. Never take Paracetamol to treat a hangover headache — this is one of the most dangerous combinations I can warn you about.
Price comparison — are you paying extra for a brand name?
Yes — you often are. Let me be honest about this.
Dolo 650 and Crocin are both branded Paracetamol tablets. Their prices are higher than generic Paracetamol. In government hospitals and Jan Aushadhi Kendras, you can get the exact same 650mg Paracetamol for a fraction of the price under its generic name.
The quality is regulated by the same CDSCO standards. A generic Paracetamol 650mg from a licensed manufacturer is pharmacologically identical to Dolo 650. The only difference is the brand recognition and the marketing spend behind it.
I am not saying branded medicines are bad. I am saying — if cost is a concern for you, especially for long-term use, ask your pharmacist for a generic alternative. It is the same medicine.

My final verdict as a pharmacist
After years of dispensing both these medicines every single day, here is my honest assessment:
Dolo 650 is not better than Crocin. Crocin is not better than Dolo 650. They are both Paracetamol. Choose based on the dose you need — not the brand name.
If you are an adult above 60kg with significant fever or body pain — take 650mg Paracetamol. Use Dolo 650 or Crocin 650 — they are identical at this dose.
If you have mild fever or a headache and you are on the lighter side — Crocin 500 is perfectly adequate and puts less Paracetamol load on your liver.
If you have a tension headache — consider Crocin Pain Relief for the caffeine boost.
And if cost matters to you — ask your pharmacist for a generic 650mg Paracetamol. Same medicine, lower price, no loss in effectiveness.
The real winner in the Dolo vs Crocin debate? Paracetamol. It has been the world's most trusted fever and pain medicine for over 60 years — regardless of what brand name is printed on the strip.
Frequently asked questions
Can I take Dolo 650 and Crocin together?
No. Both are Paracetamol. Taking both together means double dosing Paracetamol which can damage your liver. Never take two different brands of the same drug simultaneously.
Dolo 650 COVID mein kyu itna popular hua?
During COVID, doctors recommended Paracetamol 650mg as the safest first-line treatment for fever at home. Dolo 650 happened to be the most widely available 650mg brand at the time, and the name stuck in public memory. The medicine itself — Paracetamol — was the recommendation, not any special property of the Dolo brand.
How long should I take Paracetamol for fever?
If fever continues for more than 3 days despite regular Paracetamol, you must see a doctor. Paracetamol manages fever — it does not treat the underlying infection causing it. A persistent fever needs investigation, not just more tablets.
Is Dolo 650 safe during pregnancy?
Paracetamol has traditionally been considered the safest painkiller during pregnancy. However recent studies have raised some concerns about prolonged use during pregnancy. Always consult your gynaecologist before taking any medicine during pregnancy — including Paracetamol.
What should I do if I accidentally took too much Paracetamol?
If you have taken more than 4 grams (4000mg) in 24 hours — especially with alcohol — go to a hospital immediately even if you feel fine. Paracetamol overdose damages the liver slowly and symptoms may not appear for 24 to 72 hours. By the time symptoms appear, the damage can be severe. Early treatment with N-Acetylcysteine can prevent permanent liver damage if given quickly.
